Community Development Program

Community development is the collaborative, collective action taken by local people to enhance the long-term social, economic, and environmental conditions of their community. The primary goal of community development is to create a better overall quality of life for everyone in the community.  The LPCA Community Development Program assists communities in the development of new primary care access points, or clinics, to serve medically uninsured and underserved populations throughout Louisiana, primarily Federally Qualified Health Centers (FQHC).
 
This program includes development of the Statewide Strategic Plan, a database of demographic and health information which is used to develop and refine the long-term, statewide Community Health Center growth plan for Louisiana.  The SSP is then utilized to guide the expansion of new sites, satellite locations, and services of established Community Health Centers (CHCs).  
 
Background
How LPCA Works With Communities
 
The process is different in every community (Memorandum of Understanding) because every community is unique. LPCA works hand-in-hand with the Department of  Health and Hospitals Bureau of Primary Care and Rural Health, the Health Services Resources Administration (HRSA), the Bureau of Primary Health Care (BPHC) and the National Association of Community Health Centers (NACHC) to offer services and assistance to 1) educate and mobilize the public; 2) work with communities in early stages of community change; 3) create new access points; strengthen the safety net; and 4) increase types of services available to communities. The community development staff works conscientiously to:
  • Identify, organize and convene Community Leaders – help you communicate with those concerned about primary health care access and willing to take action; establish partnerships with local coalitions and community leaders; conduct community outreach through media and other venues; and develop leadership potential.
  • Strengthen the Safety Net – develop initiatives to sustain and strengthen safety providers and encourage joint planning and information-sharing; and work with communities to create integrated delivery systems through coordination and collaboration.
  • Provide referrals and resources to provide statistics and assessment of community needs such as the state’s Bureau of Primary Care and Rural Health, and national assistance through the Health Services Resources Administration (HRSA), the Bureau of Primary Health Care (BPHC) and the National Association of Community Health Centers (NACHC).
  • Enhance Boards Through Comprehensive Training - The Louisiana Governance Academy provides a 30-hour series of workshops to move your board of directors to excellence in governance.
  • Help Communities Understand Delivery Systems and Models – define different alternatives to address health care needs, assess appropriateness of FQHC health care delivery model for your community.
  • Facilitate Partnership Development & Coalition Building - identify, educate and motivate potential partners who can assist.
  • Identify Various Financial Resources – assist with identification of fundraising and grant proposal writing resources such as Community-Based and Rural Health Program , and providing training on the community development process and 330 grant proposal writing.
  • Identify consultants to assist communities as needed330 Proposal Writing.
  • Comprehensive Assistance With Regulatory and Legislative Guidelines - we can help you navigate the federal assistance guidances and regulatory expectations.
  • Developing new federally qualified health centers (FQHC)Federally Qualified Health Center.
  • Mentoring and Support Services for Newly Established Community Health Centers.
  • Provide resources information to develop grant proposals such as state and parish data Data and Statistics and other FQHC information such as REACH Data, Uniformed Data System (UDS) and National Statistics.
  • Provide letters of support to organizations seeking Section 330 funding or Look-Alike Status.
