This session will explore the critical role of non-medical barriers to care in shaping patient outcomes. Participants will learn how to implement standardized screening tools, including PRAPARE, within Federally Qualified Health Centers (FQHCs). The training will cover best practices for integrating SDOH screening into clinical workflows and strategies for connecting patients to community resources.

Learning Objectives:

  1. Explain the impact of SDOH on population health and patient outcomes.
  2. Describe the purpose and components of the PRAPARE tool and other SDOH screening instruments.
  3. Identify practical steps to integrate SDOH screening into routine workflows.
  4. Apply strategies for effective referral and linkage to community-based resources.

Kelseanna Hollis-Hansen, PhD, MPH, is a faculty member at the Tufts Friedman School of Nutrition Science and Policy and the Food is Medicine Institute. She serves as Director of the New England Regional Collaborative for Food is Medicine (NERC) and Co-Director of the Healthy Eating Research (HER) Early-Stage Investigator Working Group. As a global consultant, she advises healthcare systems and nonprofit organizations on strategies to integrate strategies that address non-medical drivers of health and nutrition into patient care. For over 15 years, Dr. Hollis-Hansen has developed and evaluated community and health system interventions that address non-medical drivers of health. Her work focuses on pairing direct access to nutritious food, such as medically tailored meals, groceries, meal kits, and SNAP/WIC enrollment, with behavioral strategies to support healthy eating, improved patient outcomes, and household resiliency.
