LPCA’s Annual Continuing Education Conference is open to all organizations and individuals who have a stake in the Community Health Center movement and share our passion for making affordable, quality healthcare accessible to every Louisiana community.

This year’s theme is “Pressing Forward with Excellence: Winding Down Pandemic Practices, Accelerating into the Future of Healthcare” – a nod to the challenges we have faced but our will to overcome and continue our life-saving work on behalf of our patients.

2023 Community Health Care Excellence Awards
The Louisiana Primary Care Association invites you to submit nominees for the 2023 Community Health Care Excellence Awards. These awardees will be featured at Awards Banquet on Wednesday October 18, 2023.
DEADLINE FOR SUBMISSIONS IS FRIDAY, AUGUST 4, 2023 at 5 p. m.
Note: All nominees must be affiliated with a Louisiana Federally Qualified Health Center and in good standing with LPCA. The only exception to this is the Legislative Champion of the Year Award. PLEASE COMPLETE THE SECTIONS BELOW FOR WHICH YOU HAVE A NOMINEE. This form can be used for several nominations - if possible please only submit this form once and include all your nominees in one submission. However, if necessary this form can be completed more than once.

Eric B. Taylor Leadership Award
This award is presented to an outstanding staff member of a Louisiana Federally Qualified Health Center who exemplifies passion and commitment to improving the care and quality of life for health center patients.
Customer Service Excellence Award
This award recognizes outstanding contributions by front line and other health center staff who have direct interaction with health center clients.
Consumer Advocate of the Year Award
The Consumer Advocate of the Year inspires others to make a difference by engaging their elected officials in discussions and promoting policies that support the work of FQHCs.
Legislative Champion of the Year Award
The Legislative Champion of the Year Award goes to an individual who serves as an elected official in the public policy arenas of local, state or federal government who has gone above and beyond championing wider access to primary care.
Healthcare Provider of the Year Award
The Health Care Provider of the Year Award recognizes an outstanding health care provider who has demonstrated dedication, expertise and concern for their patients in a compassionate and comprehensive manner.
