Lessons Learned: Implementing and Expanding Social Needs Screening Programs in Health Centers

If you are part of EHR or IT staff, clinical or operations staff, leadership, quality, and population health staff, and/or clinical staff, this virtual learning collaborative is for you!

The HITEQ Center has partnered with the Louisiana Primary Care Association to design this learning collaborative to learn about health center promising practices and key considerations to support the successful collection, monitoring, and addressing of social needs data. During the series, participants will explore the levels of maturity in the social needs screening implementation process. The levels of maturity include:

Level 1: Coming to Consensus
Level 2: Implementing a Social Needs Screening Tool
Level 3: Responding to Positive Screens
Level 4: Monitoring and Using Data
Gain information on concrete strategies and IT solutions that will help to improve internal systems, such as EHR utilization and care team workflows, and increase their capacity to advance individual and population-level health.

This collaboration will begin January 26th, 2023. All sessions are scheduled for Thursdays, starting at 12 PM ET and ending at 1 PM ET.

Sessions will be held on the following dates:
January 26, 2023 – Introduction & Coming to Consensus
February 16, 2023 – Level 2: Implementing a Social Needs Screening Tool
March 9, 2023 – Level 3: Responding to the Social Needs Screening
March 30, 2023 – Level 4: Monitoring Population Level Data and Beyond
April 20, 2023 – Learning Lab: Health Center Sharing on Actions and progress in Implementing and Advancing Their Social Needs Screening Program
**Health centers interested in participating in the learning collaborative series can submit ONE registration form on behalf of their health center; including up to four participants in their form. All participants listed will receive a confirmation email and calendar invites to the series. There is NO fee to participate and this learning collaborative will be exclusively for health centers.**
