FY25 New Access Points (NAP) NOFO
New Access Points (NAP) funding helps expand affordable, accessible, and high-quality primary health care for underserved people. These grants are the main way that organizations become funded health centers in HRSA’s Health Center Program.
NAP funding targets the nation’s high-need geographic areas and medically underserved populations, including low-income and uninsured people. HRSA uses the Unmet Need Score to assess areas of greatest need for services

Uniform Data System Report
HRSA's Bureau of Primary Health Care's Uniform Data System (UDS) tracks a variety of information, including patient demographics, services provided, clinical indicators and utilization rates. UDS data are collected from HRSA health centers and reported at the grantee, state, and national levels. The data are used to improve health center performance and operations and to report overall program accomplishments. The Uniform Data System (UDS) provides annual data snapshots, demographic trends, and data comparison tools which allow for state-to-state and state-to-national comparisons. Data is presented in a variety of tables and charts, and can be exported into Excel to facilitate public use.
Economic Impact Reports
Produced annually, economic impact reports measure the effect a health center's current operations and future growth have on the community it serves. LPCA and member health centers use these reports to educate policymakers, seek community support, and pursue funding opportunities.
Every year, we work with Capital Link to provide this important reporting to our members.
LPCA Annual Reports
Every January, we publish LPCA's Annual Report in order to educate our members, healthcare stakeholders, elected officials, and the general public about the work of LPCA and Louisiana's Community Health Centers. From clinical quality measures, to information about programs and events, the LPCA Annual Report provides a comprehensive look at our organization and our mission to create a healthier Louisiana.
HCP GeoCare Navigator
The HCP GeoCare Navigator is designed to help inform users about the current geographic extent of U.S. federal (Section 330) Health Center Program (HCP) grantees and look-alikes and was largely designed upon algorithms and reporting methods developed by John Snow, Inc. for BPHC-requested service area analyses.
The information available in the HCP GeoCare Navigator includes estimates of the collective service area of these health centers by ZCTA, including the ratio of HCP grantee and look-alike patients reported in the Uniform Data System (UDS) to the target population, the change in the number of those reported patients over time, and an estimate of those in the target population that remain unserved by HCP grantees and look-alikes reporting data to the UDS (but may be served by other providers). This is a FREE resource.
National Data from NACHC
The National Association of Community Health Centers collects data, performs analysis, and applies information to empower communities in their efforts to improve public health. NACHC also educates the public, health officials, and decision-makers at the local, state, and national level about the critical role of health centers in promoting access to high quality, affordable health care that reduces disparities and advances community well-being.
